Care Coordination offered by Catholic Community Services' African American Elders Program

Provides short-term assistance with helping individuals understand the resources available in the community that might best address their needs, then helping the person in coordinating their services so they are able to continue to live independently. Staff ask questions to develop specific goals to create a plan and partners with clients to implement the plan. Clients are seen in their own homes.

Community Living Connections specialist must complete and email the form to a CLC lead.

Eligibility

African-American adults ages 60 and older or adults ages 18+ with a disability. Must self-identify as African-American, Black or African. Cannot serve those on the Eastside and NE King County including Bothell and Kenmore. Serves clients living in Renton.   Must require multiple services and/or related activities performed on their behalf; and are unable to obtain the required services and/or perform the required activities for themselves; and do not have family or friends who are able and willing to provide adequate assistance. Focuses on low-income elders in Central and Southeast Seattle and King County, especially those at risk of premature institutionalization.   Those who require an interpreter must except someone from the CCS Language Bank, which cannot include a family member or friend. Does not serve those living in an assisted living facility, adult family home or skilled nursing facility. However, those living in senior residential communities (independent living) are eligible. Cannot serve clients receiving long-term care services from the state (such as COPES). Must not currently be receiving case management through Home and Community Services or a housing authority.

The African American Elders Program serves African American elders in Central and Southeast Seattle and South King County. The program identifies frail, isolated and hard-to-serve African Americans and assists them in accessing and receiving needed social and health services through ongoing case management, support and referrals.
