ACT - Assertive Community Treatment offered at Kitsap Mental Health Services

ACT offers rehabilitation and support services to adults experiencing severe and persistent psychosis. During the screening process, our ACT team will determine which model of care best meets the individual's needs. We offer two models of care under the ACT umbrella: Program of Assertive Community Treatment (PACT) and Flexible Assertive Community Treatment (FACT). Regardless of the model of care, our full team of experts is available to provide individualized support. This team can support career and learning goals, offer peer support and mentorship, provide access to preventive healthcare, help develop healthy lifestyle choices, coordinate with medical services, and provide medication treatment and education.

Eligibility

Kitsap County residents 18 years or older who have been referred by a mental health professional or hospital, go through the screening process, and are experiencing significant impairment in one or more areas of life as a direct result of psychosis symptoms. Must have a diagnosis of schizophrenia, or schizoaffective, schizophreniform or other chronic psychotic disorder. The primary cause of their psychotic symptoms cannot be known to be from the temporary effects of intoxication, major depression or other medical conditions.
