Eligibility
Chinese-speaking adults ages 60 and older or adults ages 18 and older with a disability. Must require multiple services and/or related activities performed on their behalf; and are unable to obtain the required services and/or perform the required activities for themselves; and do not have family or friends who are able and willing to provide adequate assistance. Does not serve those living in an assisted living facility, adult family home or skilled nursing facility. However, those living in senior residential communities (independent living) are eligible. Cannot serve clients receiving long-term care services from the state (such as COPES). Focuses on those who need assistance with managing personal care and daily living tasks, are homebound or physically isolated due to health reasons as well as those who are frail and need cognitive support. Must not currently be receiving case management through Home and Community Services or a housing authority.

Provides short-term assistance with helping individuals understand the resources available in the community that might best address their needs, then helping the person in coordinating their services so they are able to continue to live independently. Case managers conduct interviews to assess the clients' needs, develop culturally appropriate care plans, work in coordination with home care and service providers and monitor client progress.
