Bilingual and bicultural social workers provide support services for family caregivers. Offers: - Information and referral - Case management - Needs assessment - Service plan development - Consultation - Caregiver education and counseling - Respite care services including housekeeping and errands , personal care. Information and resources support. Services may be delivered through phone, office/home visits or other venues.

Eligibility
Unpaid adult caregivers, ages 18 and older, caring for someone age 60 and older or an adult age 18 or older with a disability. Caregiver must live in King County. Care recipient must not be on Medicaid funded long-term care services (i.e. COPES, CFC). Focus on Asian and Chinese background caregivers, especially those caring for someone who has dementia or other health conditions.

Bilingual social workers and staff provide supportive services for English, Chinese and Vietnamese speaking (through an interpreter) family caregivers. Offers home care, residential rehab and long-term care services, long-term care consultation, information and assistance, and related services. Kin On's Healthy Living Program also provides various in-person and virtual social, activity and evidence-based programs such as workshops in the areas of chronic disease self-management and fall prevention.
