Will work with poly substance users, but insists on avoidance of methamphetamine use.

Hours

M-F, 8am-5pm.

Application process

Call. Able to induce patients on medication with 4-7 business days from initial contact, typically one office visit is needed before starting patient on medication.

Fees

Accepts private insurance, Medicare and Apple Health/Medicaid.

Office based, community health clinic that offers MAT services. Able to prescribe Suboxone and Vivitrol. Can accept 2 new patients a week with private insurance, and 2 new patients a week with Apple Health/Medicaid. Can be referred to behavioral services such as individual and group counseling.
