Application process
Requests a referral from the patient's primary care provider. This provider must be willing to follow-up on the treatment recommendations of our psychiatrists, and therefore must be familiar with the patient and be practicing in Washington state. To expedite the referral process, the referral should include the patient's name, date of birth, phone number, address, and insurance information. It should also include the patient's diagnosis and symptoms and the referring provider's contact information.

Provides one-time psychiatric evaluations for adults to determine a diagnosis and recommend treatment. This clinic is the primary training site for psychiatry and psychology residents who offer time-limited, evidence-based psychotherapies to address depression, anxiety, panic, trauma, PTSD, and insomnia. Residents do no offer DBT or psychotherapy for substance use or eating disorders. Also offers consultation and treatment for women with mental health conditions who are pregnant, considering becoming pregnant, or have recently given birth.
Does not offer court orders, mandated evaluations, disability evaluations, or labor and industry-mandated evaluations.
