Offers a comprehensive medical care for children of all ages who are placed outside of the home, as well as the families who care for them. This includes: - Children in foster, group homes & adoption care - Young adults who have “aged out” of foster care - Pregnant teens in foster care - Unaccompanied minors - Youth in kinship care - Birth parents whose children are at risk or in foster care or have been recently reunified. - Each and every member of the care family that supports these children & young adults at every stage of their journey. Offers medical, dental and behavioral health services in one location, and provides referrals for specialist care and recommendations for wrap around community support. Includes a multi-disciplinary team that has experience working with the child welfare system. Services include: - Medical, dental & behavioral health care using a trauma informed lens - Full spectrum family practice including initial health screenings and Well Child exams - Quick access to meet new placement needs - Unique sensory sensitive space - Sacred Patient Time—longer appointments - Coordinated care with case workers - Transitional care through permanency - Specialty referrals as needed - Innovative community approach. - Pharmacy and lab services on site
