Compass Care Coordination offered at iMPACT! Compassion Center
Provides a comprehensive/holistic program that offers a wide variety of life skills education/guidance and support services for Medicaid recipients. Clients work directly with a care coordinator to identify barriers and goals. Examples include parenting, foster parenting, at-risk youth, family reconciliation, money management, healthy relationships/domestic violence, trauma/abuse, special needs, health/medical, school issues, housing/homelessness, employment, mental health management, communication skills, problem-solving/goal-setting, setting boundaries, and navigating community resources. Provides hands-on tools and guidance and connect people with area/regional resources.

Clients learn fundamentals life skills and receive case management services for housing/homelessness and employment. Offers comprehensive care coordination, helping individuals and families gain tools and life guidance. Provides individual, couple, and family counseling. Offer educational opportunities through workshops/support groups ad activities to promote self-sufficiency and strength. Also provides essential household food, hygiene, cleaning, and baby supplies; as well as homeless care kits.
