Medicaid-eligible children and youth up to age 21.

M-Th, 8am-7pm F, 8am-5pm

Application process

Call to schedule a screening, either over the phone or in person. Youth over age 13 can request screening and must consent to services if screening is requested by a parent.

Provides individualized, ongoing support for Medicaid-eligible youth with complex behavioral health needs and their families to help avoid or reduce costly and disruptive out-of-home placement. Offers the opportunity for parents and children to connect outside of a clinical setting with caring professionals and understanding peers who have been through similar situations. Services are available in home and in community settings. Teams typically consist of
