Intake

Application process
Call for an intake screening. Once screened into the program, clients will be routed to their local hub based on county. The hub will reach out to the client usually within a week.

Connects people to care coordination support by sharing their information with one of the Community Care Hubs of Washington that serves the area where they live. The hub reviews their needs and then assigns a trusted helper in your area to support them and connect them to the people and resources they need. The trusted helper can help them understand their options, navigate and access services, and connect with programs, resources, and people that may be able to help meet their needs. Community Care Hubs are not a direct service provider and cannot guarantee services like housing or financial assistance, but they can walk clients through the process by identifying resources, applying for services, getting on waitlists, or scheduling and attending appointments.
