Care Navigation offered by Community Care Hubs
Helps families and individuals connect to available resources. Resources available include housing, health insurance, child care, food assistance programs, and other kinds of services. The services available differ by area. Services are provided at eight regional hubs. Local care coordinators work with a person to help them apply for programs such as unemployment benefits, subsidized housing or child care, food assistance programs such as SNAP or WIC, Apple Health, and more.

Application process
Call for an intake screening. Once screened into the program, clients will be routed to their local hub based on county. The hub will reach out to the client usually within a week.
