Provides primary care, management of acute and chronic illness, diagnostic testing, well adult exams and physicals.   Other services include dietician consultations, psychiatric consultations, health education, medication management and recommendations for continued psychiatric care. A social worker helps patients access community services and resources and assists in resolving problems that interfere with patients' access to health care.
