Description

A person-centered recovery oriented mental health service, which is delivered by a transdisciplinary mental health staff in the community. For people with severe and persistent mental illness who also experience difficulties with daily living activities and tasks, and have not benefited from traditional programs. The PACT team consists of a team leader, psychiatrist, registered nurses, mental health professionals, peer specialist, vocational specialist and chemical dependency specialist to holistically meet the complex needs of the client.
