Provides care coordination for the Apple Health eligible population who are in out-of-home placement through the Department of Children, Youth, and Families. Offers - Help finding local Medicaid providers - Explains Medicaid coverage the client qualifies for - Help with prescriptions and prior authorizations issues - Answers health and behavioral/mental health questions
