  • General Disabilities
Case Managers assess the temporary and permanent needs of older and disabled adults who are receiving Medicaid in-home personal care services to coordinate these services in order to maintain maximum independence. Case Managers:
  • Assess needs of individuals on State-funded Medicaid personal care services,
  • Develop care plans,
  • Authorize and arrange services,
  • Coordinate provision of services among providers, and
  • Follow-up with re-assessments as needed.
