Provides transportation thru Access and Transia. Contracts with Nikkei Manor transportation.
Eligibility
Age 55 or older In need of nursing home level of care as defined by the state of Washington Residing in the PACE service area at the time of enrollment Able to live safely in the community with PACE services Medicaid-eligible and/or willing to pay private fees
Hours
M-F, 8am-4pm.
Application process
Call to schedule an appointment with an enrollment specialist. An interdisciplinary team will review the patient's medical history to help determine if PACE is a good fit.
Fees
Accepts Medicare, Medicaid and private pay.

Provides individualized services to keep nursing-home eligible individuals living independently at home or in the community as long as possible. A team of eldercare experts does an assessment, designs a comprehensive personalized care plan, then provides and coordinates services to address health and personal care needs. PACE health services include the following as needed:

  • Caregiver support
  • Transportation to the facility
