Works with clients to address areas in life impacted by mental illness by identifying their needs, developing an individualized treatment plan, and monitoring and evaluating progress over time. Incorporates client-centered care through teaching, modeling, and practicing skills to increase self-sufficiency and independence within various community settings. Including assistance in daily life activities, social and personal growth, financial and legal issues, medication, symptom management, and scheduling and attending medical, dental, and psychiatric appointments. Assists in connecting clients to resources such as referrals to medical/psychiatric/dental care, housing, substance use treatment, therapy, transportation, and employment.
