Children birth through 17 years of age with a condition that may prevent the child from growing, developing, or playing like others or that puts the child at risk of becoming disabled.

Description

Case management and care coordination services for families of children with special health care needs. The program provides information on childhood issues like eating and nutrition, growth and development, and behavior. The can help families in finding financial assistance, schedule appointments and transportation, get services for their child, such as medical care, and find information about health insurance programs.

The fiscal agent for publicly funded mental health, chemical dependency, housing and developmental disability/supportive employment services.  Also provides quality assurance, provider network support and technical assistance for contracted agencies.  Convenes community planning processes to maximize the return on investment of public behavioral healthcare funding.
