Application process
Call the Referral line. Program requires enrollment at least 45 days before child's 3rd birthday.
Before entering the Early Intervention program the child receives a multi-disciplinary team evaluation.
Based on the evaluation, the child is made eligible for program services and an Individual Family Service Plan is created.
