- Consent to home, video, or telephone visits
- Have poorly controlled, moderate, or persistent asthma
- Are between ages 6-12

Helps children and adults have more days without asthma symptoms, fewer emergency room visits, and fewer missed days at school or work. Offers:
- Four customized sessions with a community health worker specializing in asthma care
- A home environment assessment with a tailored plan to reduce asthma triggers in the home
- Asthma trigger reduction supplies, including a HEPA air filter
- Asthma education and resources
