Must live in Benton/Franklin County or in the city of Burbank, be uninsured, and have an income at or below 200% of federal poverty level. Does not treat the following conditions: - Chronic Pain - Routine Pap Smears - New or Ongoing Cancer Treatment - Hormonal Disorders - Blood Clotting Disorders Does not prescribe the following mental health medications or other medications similar to these: - Ambien - Adderall - Lithium - Ritalin - Xanax - Valium

Call for new patient screening and for an appointment. May be able to accommodate same day appointments for medical services only (please call to confirm). Dental and counseling sessions are by appointment only.
