Options Counseling offered by Catholic Community Services' African American Elders Program

Provides guidance to individuals to help them make informed choices about supports and services in order to live independently in the community.   Works with the individual to explore options that can help them live independently longer in a manner that is safe and meaningful to them.   An options counselor can meet with the older adult one on one to:    -  Develop a personalized action plan -  Assesses and refers to additional resources -  Provides limited assistance in filling out paperwork -  Can provide limited phone-based advocacy to connect client to resources   - Guide clients to services to meet their needs.

Application process

Call to make a referral. Accepts referrals from Community Living Connections specialists and those calling on client's behalf. CLC specialist must allow clients the option of calling themselves and may not call on behalf of a 3rd party. CLC specialists may only call on behalf of a client for whom they have obtained direct consent. If no answer, leave a voicemail with as much information about client as possible including: -  First and last name -  Phone number -  Address -  Date of birth - Monthly Income - Veteran Status -  Reasons Options Counseling may be needed -  Description of services needed -  Disability information for clients who are 59 and younger

Eligibility

African-American adults ages 60 or older and adults ages 18 and older with a disability. Must self-identify as African-American, Black or African. Not currently receiving case management through Home and Community Services or a Housing Authority.   Cannot serve those on the Eastside and NE King County including Bothell and Kenmore. Focuses on low-income elders in Seattle and South King County, including Renton, especially those at risk of premature institutionalization. Targets low-to-moderate income individuals who are isolated due to geographic barriers, lack of family support or have difficulties maintaining independent lifestyles.   Those who require an interpreter must except a person from the CCS Language Bank, cannot use a family member or friend. Those that need guidance on making care choices, but are capable of connecting to provided services on their own.

The African American Elders Program serves African American elders in Central and Southeast Seattle and South King County. The program identifies frail, isolated and hard-to-serve African Americans and assists them in accessing and receiving needed social and health services through ongoing case management, support and referrals.
