Eligibility

Clients must enroll in primary care services prior to being connected with MAT services. Youth ages 13-18 and pregnant women are accepted on a case by case basis.

M-Th, 8:30am-5pm F Sa, 8:30am-5pm.

Accepts Medicaid (CHPW and Molina), Medicare, private insurance, and self-pay. We also have a sliding fee discount scale for those who apply and qualify.

Provides suboxone and vivitrol medications in a medical clinic setting. Must enroll with a PCP first. Induction typically occurs within 1-4 weeks after initial phone contact.
