Eligibility
Families of children ages birth to 18 with a chronic health condition regardless of income. Examples of health conditions include:
- Autism Spectrum Disorder (ASD)
- Asthma
- Cerebral Palsy
- Down Syndrome (Trisomy 21)
- Cleft Lip and/or Palate
- Cystic Fibrosis
- Prematurity
- Gross / Fine Motor Delays
- Speech / Language Delays
- Social / Emotional Delays

Provides support to families of children with a chronic health condition. Helps ensure that the child and family have the resources and support they need. Services may include:
- Listening to parent concerns and helping navigate available resources
- Connecting families with local resources for early intervention services – physical therapy, occupational therapy, speech therapy, and preschool.
- Supporting parents with specific areas of concern, such as nutrition, feeding, growth, development and behavioral issues.
- Providing connections to parent support organizations.
