Eligibility

Families of children ages birth to 18 with a chronic health condition regardless of income. Examples of health conditions include:
  • Autism Spectrum Disorder (ASD)
  • Asthma
  • Cerebral Palsy
  • Down Syndrome (Trisomy 21)
  • Cleft Lip and/or Palate
  • Cystic Fibrosis
  • Prematurity
  • Gross / Fine Motor Delays
  • Speech / Language Delays
  • Social / Emotional Delays

Hours

8am to 5pm from Monday through Friday

Provides support to families of children with a chronic health condition. Helps ensure that the child and family have the resources and support they need. Services may include:
  • Listening to parent concerns and helping navigate available resources
  • Connecting families with local resources for early intervention services – physical therapy, occupational therapy, speech therapy, and preschool.
  • Supporting parents with specific areas of concern, such as nutrition, feeding, growth, development and behavioral issues.
  • Providing connections to parent support organizations.
