Eligibility

Children, ages birth through 18, with a high level of nursing needs, pulmonary impairment, or similar disability who require ongoing nursing care but do not require hospitalization. Those ages 18-20 may be served if in extended foster care.

Hours

M-F, 9am-5pm.
