Children ages birth-3. Must live in Seattle city limits, Skyway, North Tukwila (north of S 120th St), or North Burien (north of SW 148th St).

M-F, 8:30am-5:30pm.
Application process
Call for appointment.   No application for assessment necessary.

Insurance may be billed.   Required to bill co-pays, deductibles and co-insurance.
