Application process
Call for more information or to refer a client. Serves clients in 7 counties through a network of Care Coordinating Agencies. Agencies are chosen by the client preference, location and other specific needs. Current Care Coordinating Agencies are: - Coastal CAP - Community Action Council of Lewis, Mason and Thurston Counties - Gather Church - Lower Columbia CAP - Lifelong Connections - Love Overwhelming - Mason Health - Physicians of Southwest WA - SeaMar Community Health Centers - Youth and Family Link.
Eligibility
Priority population includes those who can answer yes to these three questions: 1. Does the person have a behavioral health concern (like a mental health or substance use issue)? 2. Is there an additional concern (like pregnancy, chronic disease, co-occurring behavioral health)? 3. Are there additional risk factors (like housing insecurity, recent release from a hospital, frequent need to use 911, other barriers to health). Counties served are Cowlitz, Grays Harbor, Lewis, Mason, Pacific, Thurston and Wahkiakum.

CHOICE Regional Health Network
CHOICE is a nonprofit collaborative that aims to improve community health. CHOICE provides COVID-19 Vaccine and Influenza Vaccine Education and Outreach to all community members. CHOICE also partners with influencers in the seven county region it serves to provide information and reduce vaccine hesitancy. CHOICE has a dental program that connects children ages 0-5 who are on Apple Health for Kids (Medicaid) to a dental provider. CHOICE also administers the Navigator program, and can provide referrals to individuals who need assistance in completing an insurance application through Washington Healthplanfinder.
