Foundational Community Support Housing Services offered at Community Action Council of Lewis, Mason & Thurston Counties

Eligibility
FCS (Flexible Community Supports) is available to individuals enrolled in Apple Health (Washington State Medicaid) who have significant health-related needs identified through a qualifying assessment. Eligible participants must also meet specific age requirements and demonstrate at least one high-risk factor, such as chronic homelessness, complex behavioral health needs, co-occurring mental health and substance use conditions, or a disability or long-term care need. FCS is a targeted Medicaid benefit that prioritizes individuals with the most pressing needs for housing or employment stability, rather than serving all Medicaid recipients. To participate, individuals must go through a formal referral process and receive approval from the state’s FCS administrator, which is currently Wellpoint.
