Eligibility
Serves adults ages 18 and older with severe and persistent mental illness. Must be King county Resident, income below 200% of Poverty level, fully integrated Managed Care through one of the five Managed care organizations, and have a severe and persistent mental illness.

Outpatient mental health treatment program offers case management, counseling, psychiatric services, medication management, medication monitoring, and a peer support day program. Day program provides services such as: - Member prepared daily lunches - Weekly food bank - House jobs - Holiday parties - Camping trips - Various activity groups - Group treatment. Case management assists clients in obtaining social services, provides crisis intervention, individual and group counseling, outreach and office based services, psychiatry and medication management. Collaborates with apartment managers, government agencies and other treatment providers to assure for coordination of care and basic needs. FOR TIER-ELIGIBLE CLIENTS ONLY.
