Care Coordination offered by Jewish Family Service in Seattle
Provides short-term assistance with helping individuals understand the resources available in the community that might best address their needs, then helping the person in coordinating their services so they are able to continue to live independently. Staff ask questions to develop specific goals to create a plan and partners with clients to implement the plan.

Eligibility
- Russian-speaking adults ages 60 and older or 18 and older with a disability, residing anywhere in King County and only non-Russian speaking individuals that live in East King County (ZIP codes 98004, 98005, 98006, 98007, 98008, 98015, 98011, 98014, 98019, 98024, 98027, 98028, 98033, 98034, 98039, 98045, 98050, 98052, 98053, 98065, 98068, 98073, 98074, 98075, 98083, & 98288). - Adults ages 60 and older or adults ages 18 and older with a disability. - Must require multiple services and/or related activities performed on their behalf; and are unable to obtain the required services and/or perform the required activities for themselves. - Serves those who need assistance in accessing services and do not have a friend, relative, social worker or other professional who are able to help them understand their options or access the services. - Does not serve those living in an assisted living facility, adult family home or skilled nursing facility. However, those living in senior residential communities (independent living) are eligible. - Cannot serve clients receiving long-term care services from the state (such as COPES). - Must not currently be receiving case management through Home and Community Services or a housing authority.
