Eligibility
Any patient who has been funded either through the Komen or the Cancer Lifeline fund in the past is not eligible to apply again.
Must be considered to have an urgent need such as:
- Exhausted services from other agencies for food

Application process
Electronic application ONLY.
Work with oncology social worker or healthcare worker to fill out the online application form.
May take up to two to three months, so program is not able to assist with immediate shut-off or eviction notices.
Funds are limited and applications will be processed on a first-come, first-served basis according to availability of funds. Due to limited fund availability, not all eligible applications will be served.
Cancer Lifeline PAF may only be awarded one time.
