Eyeglasses Voucher offered by New Eyes

Eligibility
  • Monthly income at or below 250% of Federal Poverty Level
  • Eyeglass prescription dated within the last 24 months
  • No other resources available to pay for glasses, including federal/state programs or assistance from local charitable organizations
  • Have not received a New Eyes voucher in the past 12 months

Application process
Application should be submitted online by a social worker, case manager, or an agency on behalf of their client. Also accepts applications from individuals in financial need but must be able to prove eligibility. If the application is approved, a unique e-voucher number is emailed with a link to the eyeglass ordering site and instructions on how to order eyeglasses. Approved applicants must order their glasses through New Eyes' online eyeglasses dispenser.
Fees
$15 application fee for all applicants. Lens upgrades such as progressive, tinted, blue-light filter, etc. are available at discounted, out-of-pocket cost.
