Call the admissions coordinator. Referrals are accepted from Medical Case Managers/HIV providers.

Provides a skilled nursing facility for people with HIV/AIDS; physical rehab, long-term care and end-of life care for those with complex conditions including ALS, Huntington's disease and cancers. Outpatient program supports individuals with HIV/AIDS offering medication management, chemical dependency and mental health outreach services. Emergency Homeless Shelter and housing stability programs for those enrolled.
