Eligibility

  • Must be between 18 and 69 years of age;
  • weigh at least 110lbs;
  • have a valid photo ID;
  • proof of SSN or INS number;
  • proof of permanent address. This may include rental lease agreement, or mail post marked within the last 30 days.

M-F, 6am-6pm Sa, 9am-3pm

Application process

Calling to schedule an appointment is preferred and faster, but drop-ins are accepted.
