Eligibility

- Must be an existing client of Developmental Disability Administration, Division Vocational Rehabilitation, or Foundational Community Services, or - Must be 16 and older, receiving Medicaid, and diagnosed with long-term substance use disorder or dual diagnosis, or - Must be needing assistance with a minimum of 3 ADL’S (Activities of Daily Living) or 1 hands on ADL that may require body care, or - Enrolled in the state housing and essential needs (HEN) or aged, blind or disabled (ABD) program.

Description

Provides employment and training services for individuals 16 and older with chemical dependency, disabilities, or any other physical struggle. Helps individuals prepare and enter the workforce by tailoring to their needs, interests, and skills. Tries to match each individual with the right employer. Provide a wide range of services to support individuals find pathways to a successful career, including employment planning, vocational assessment, job placement, job development, job training, and job retention and more. Works with the Developmental Disability Administration, Division Vocational Rehabilitation, and Foundational Community Services.
