Provides medication assisted treatment for substance use disorder for patients through a spoke and hub access model. Intake includes a full assessment for SUD treatment and creating an individual treatment plan. Treatment program will include referrals to other partner agencies for services including discharge planning, behavioral health services and may include other physical health providers.
