Provides primary health care services such as medical, preventive health and dental, substance abuse treatment and counseling, and mental health services to people experiencing homelessness. Offers case management services, including assistance with entitlement applications, translations, housing, employment, and job-training referrals. Clients also may be eligible at times for bus, shower and food vouchers. Additionally offers
  • Prescription assistance: medical prescription assistance to low-income clients/patients, or those who have no income or insurance to cover the prescriptions.
  • Shelter Outreach Services: medical and case-management services at local shelters.
  • Migrant Outreach Services: medical and health outreach and assistance for migrant populations during summer months.
  • Referrals to community agencies for assistance with food, clothing and household items and other services.
