ONLY SERVES OKANOGAN COUNTY. - Adults ages 18 or older - Must have a behavioral health diagnosis (mental health and/or substance use disorder) - Must be homeless or at risk of homelessness, institutionalization, or involvement with the justice system - Must be enrolled in or eligible for behavioral health services through a BH-ASO or MCO - Must be willing to engage with supportive housing case management Medicaid is not required. Income documentation may be needed. 
M-F, 8am-5pm.

Provides long-term rental assistance to eligible individuals in Okanogan County who have behavioral health needs and are homeless or at risk of homelessness. The program supports housing stability through rental subsidies paired with ongoing case management and support services. Participants must engage in behavioral health treatment and work with a care team. Services are intended to help clients avoid institutionalization, reduce crisis service use, and maintain stable housing.

As a non-clinical entity, it maintains close affiliations with community health centers throughout the region. The mission is to facilitate comprehensive care coordination through a team of community-based care coordinators who posses hand-on experience with local resources across North Central Washington. Coordinators are trained as generalists and specialists, enabling them to support the diverse needs of community members. Prioritizes hiring individuals with lived and living experience to foster relational connections and human support, empowering individuals as they navigate services essential for daily life, personal, challenges, and growth.
