Varies depending on services needed. A short, confidential phone screening will be conducted by a member of the care coordination team. Once complete, you will be referred to a screened, “best-fit” provider to meet your needs.

Description

Connects low-income, uninsured individuals with donated specialty medical care through a network of volunteer providers and hospitals. Provides case management and interpretation services and accepts self-referrals for dental, behavioral health, food, housing, and other support services.
