Provides outreach-based, high-intensity mental health treatment and behavior support designed to reduce avoidable hospitalizations, homelessness, and law enforcement involvement. Depending on individual needs, services may include expanded community services (behavior support), outreach and engagement, individual therapy/counseling, case management, crisis response, and medication management. Services are delivered multiple times per week as needed, often in Adult Family Homes (AFHs) and other community settings.
