Description

Provides home health care coordination of medical, behavioral health, long-term services and other community support for eligible Medicaid clients (who also receive Medicare) at no cost. Service is provided in order to partner with clients, their families, and service providers to ensure coordination across systems. Additionally, a person-centered health action plan is developed with each client to improve their quality of life and slow the progression of chronic diseases; focus is on self-management.
