Eligibility
Serves individuals with the most severe and persistent mental health conditions who experience significant functional impairments and have not benefited from traditional outpatient services. Many individuals also have co-occurring substance use disorders, complex medical or social needs, or frequent involvement with crisis, judicial or inpatient systems.

PACT provides treatment for individuals with serious and persistent mental illness, delivering services directly in homes and community settings to support real-world stability and growth. Using a transdisciplinary team approach, staff share responsibility for care and provide goals and stage of recovery. Services include crisis response and stabilization, medication support and delivery, housing and tenancy support, employment and skill development, integrated co-occurring disorder treatment, daily living skills training, and coordination with hospitals, legal systems, and community partners. PACT focuses on reducing hospitalizations and homelessness while helping individuals build independence, manage symptoms, strengthen community connections, and achieve meaningful, self-defined recovery.
