Case Management offered by Harborview Medical Center's Madison Clinic
Facilitates social work case management services and advocates on behalf of clients seeking mental health, substance use, and medical treatment. Provides comprehensive psychosocial assessments, develops and monitors care plans, and offers service coordination, sexual health and prevention counseling, as well as back-to-work counseling and referrals. Assists clients in accessing community services and public benefits, and helps coordinate care across outpatient and inpatient settings. Additionally, provides financial advocacy by helping patients access income and medical coverage benefits, supporting benefits management, networking with DSHS and Social Security, guiding back-to-work planning, and coordinating with hospital financial counselors and healthcare coverage services.
