Provides medical care services to families and people of all ages including treatment of chronic diseases, preventive care, women's health care, and geriatric care. Creates long-term care plans. Services include: - Diagnosis and treatment of minor ailments - Yearly physicals - Infant and child check-ups - Skin biopsies - Minor office procedures - Congestive heart failure - Diabetes and endocrine care - Hypertension disease Connects patients to other CHC specialists including nutritionists, physical therapists, and behavioral health specialists as well as specialists outside the clinic. Pharmacy available onsite.
