Eligibility
Adults ages 18 and older with a diagnosis of a severe and persistent psychiatric disorder, a history of frequent crisis system contact, multiple psychiatric hospitalizations, and unsuccessful community placements.

M-F, 8:30am-5pm. Hours can vary as treatment teams provide flexible, scheduled client contacts.

Provides a home and community-based, high-intensity treatment service for individuals who could benefit from more frequent contact from their clinical team and, to avoid more restrictive levels of care such as psychiatric hospitalization, engage in an individualized treatment program that addresses specific needs. Also serves as a step down program from higher levels of care such as PACT or residential treatment. Specific services include case management, crisis response, individual therapy, medication management, and outreach and engagement.
