Eligibility

Open to those who are: - Adults aged 18 years or older AND - Unhoused or at imminent risk of losing housing AND - Has a severe substance use or co-occurring mental health disorder AND - Are unable or unwilling to access traditional behavioral health services.

Hours

M-F, 8am-5pm.

Application process

Organizations or individuals can call or email. Please provide: - Client Name - Client Identifiers - Client Date of Birth - Last Known location - Reason for referral.
