SAGE Mental Health Case Management Program offered at DESC's 216 Clinical Services
Provides a comprehensive outpatient mental health program. Case managers, psychiatrists and nurses work with single adults, most of whom are or have been homeless, in assisting them in their journey towards recovery. This includes help with obtaining and maintaining housing, improving clinical and social stability and enriching their level of independence and self-sufficiency. Services are provided in DESC offices, at client homes and elsewhere in the county such as shelters, hospitals, jails and wherever else clients’ lives take them. Services include the following: - Case management - Psychiatric services - Counseling - Therapy - Representative payee services - For some clients, assistance in obtaining employment or referral to vocational service programs SAGE services are licensed under the Washington State Division of Behavioral Health and Recovery and are provided under contract with King County Behavioral Health and Recovery Division. Services are provided within a Recovery model.

Application process
Call and request to speak with the SAGE intake specialist. Clients may self-refer. Professionals may contact the intake specialists directly to make a referral. Same-day screening for services is possible. Intake appointment will occur within ten days of the request for services.
Fee
Primarily funded by WA Apple Health (Medicaid), although clients can also pay out-of-pocket if they do not have Medicaid. Some clients may qualify for non-Medicaid funding.
Eligibility
Adults residing in King County who meet the medical necessity criteria in the King County Mental Health Policy and Procedure Manual. Clients must also be Medicaid recipients or meet the non-Medicaid criteria.
