Family Caregiver Support offered by Catholic Community Services' African American Elders Program
Provides case management, support and referral services for unpaid family caregivers of African-American adults. Caregiver support services may include: - Individualized comprehensive needs assessments and develop care plans - In-home or office visits within King county - Referrals to support groups, counseling and other resources - Limited respite for the caregiver

Call for more information. Caregiver will be asked to complete an assessment to determine the burden of caregiving.
Eligibility
Unpaid adult, family caregivers of an African-American elder, age 60 or older or an adult 18 or older with a disability. Focuses on caregivers living in Seattle and South King County. Cannot serve those on the Eastside and NE King County including Bothell and Kenmore.
