Promotes integrated systems of care that assure children with special health care needs have the opportunity to achieve the healthiest life possible and develop to their fullest potential. Services can include: helping a family find or coordinate medical providers, a medical home, help accessing local resources or services for the child and provide information about local community resources, including parent support organizations and community coalitions.

Provides support and services for patients and families living with a diagnosis of Parkinson's disease. Services include support groups, exercise and speech classes, education workshops, case management, and counseling. Offers hybrid formats, online, in-person groups, and outdoor activities.

Provides integrated behavioral health care to adults, children and families with intellectual and developmental disabilities. Creates customized treatment plans that may include mental health and substance abuse assessment and treatment, medications, and supports such as: - Individual, couple, family, or group counseling sessions - Trauma-related services - Medication monitoring and management - Nursing services - Peer support - Crisis support - Employment support

The developmental disability case manager and staff provide community access programs and supported employment to community members that have developmental disabilities. They also provide cooking and nutrition classes, craft activities and socials to encourage them to interact with others in the community.

A medical social worker specializing in cancer patients helps people - deal with feelings, - strengthen coping skills, - improve communications with family and friends, - make important decisions and - take control of their illness and lives. The counselor also makes referrals to appropriate community resources to help meet individual needs. Also does advance care planning with patients. All clinics have notaries available to notarize the documents.
