Outpatient Enrollment Center for Frontier Behavioral Health. Provides in-person assesments (Walk-ins encouraged) for those seeking services ages 0-100+. Assessments are 60 to 90 minutes and are completed M-F, 7:30am-3:30pm. Those interested in services are encouraged to call ahead to verify eligibility and availability. Staff can start assessments over the phone, but individuals MUST complete the assessment in-person or zoom.

Provides a 16 bed Evaluation &Treatment center in a secure setting, designed to provide immediate assistance with acute psychiatric problems. Services include medication management, therapy, milieu treatment, and community referral. All admissions are arranged through the DCR, and hospitals there are no self referrals. Individuals leaving the facility will have assistance with development of a discharge plan designed to sustain and build upon gains achieved while in treatment.

Provides ongoing mental health services for adults. Services include mental health therapy, case management, and crisis support for current clients during business hours. Once a client is engaged in therapy or case management, referrals for psychiatric evaluations can be made to help determine appropriate treatment, including medication management when needed. Also offers referrals to its supportive housing and substance use disorder (SUD) treatment programs, as appropriate.

ACT offers rehabilitation and support services to adults experiencing severe and persistent psychosis. During the screening process, our ACT team will determine which model of care best meets the individual's needs. We offer two models of care under the ACT umbrella: Program of Assertive Community Treatment (PACT) and Flexible Assertive Community Treatment (FACT). Regardless of the model of care, our full team of experts is available to provide individualized support.
