Provides guidance to individuals to help them make informed choices about supports and services in order to live independently in the community.  Focuses on those who need additional support around a transition or change in their lives such as facing a new health condition, recent move, or change in social network. Specialists can meet one-on-one. Works with the individual to explore options that can help them live independently longer in a manner that is safe and meaningful to them.  An options counselor can meet with the older adult one on one to:    -  Develop a personalized action plan -  Assess and refer to additional resources -  Provide limited assistance in filling out paperwork -  Can provide limited phone-based advocacy to connect client to resources   During visit CANNOT provide: -  Housework -  Errand running -  Personal care services -  Transportation -  Legal representation or advice

Federal program for navigating the Long Term Care (LTC) services and processes so that the individual may receive any desired help with meeting their personal care needs including getting dressed, driving to appointments, or making meals. Offers Pathfinder services to help find providers in the area as well as different types of services depending on special circumstances, needs, or demographics.

Provides information, assistance, and initial eligibility screening for services to support seniors, people with disabilities, and their caregivers in assessing their needs, and identifying the most appropriate service provider. Helps facilitate access to social service programs, long term care options, and provides follow-up and advocacy to individuals experiencing barriers to services to ensure needs are met. Also conducts community education and outreach for social service providers and private businesses.

The case manager stays involved to assure that the plan is implemented properly. Provides assistance when a caregiver is unable to handle increasing care needs, if a family wants to know what options they have, or because a person needing care wants to talk to someone about their options.

Helps adults with challenging behaviors transition from a hospital to a long term care facility. Team supports successful community placements and aims to prevent hospital readmission. Geriatric Consultants work with adults to create a plan based on a detailed assessment that helps both the individual and the facility.

Provides a free evaluation to find the right the care solutions and support.    We have nurse care managers and Placement Navigators to help families make the right care decision.   Placements may include meeting with a Specialist and/or Senior Living Adviser who would help families tour facilities, screen potential home care providers, and provide information on caregiver coping.   Help families determine where the best skilled nursing home or rehabilitation facility is for their needs.   Help families identify supported living options or in-home care services and helps coordinate services.   Home Care Coordination services help families understand which type of care is appropriate based on their unique needs, such as aging, chronic illness, and disabilities.   The analysis includes a projected cost for care and what services would be beneficial for long term care goals.    Also offers a free care planning support service to help a family identify and agree on the required care needs, and each family member's role in meeting those needs.   Care Management provides an evaluation of one's ability to function independently.   Care managers can provide medication and treatment management, arrange for any necessary home modification, recommend and manage support services like home care, specialty physicians, and community programs.   May also help schedule medical appointments, accompany those in their care on trips, keeps track of important care-related medical and financial records.   Facilitates ongoing communications with the entire family so everyone is aware and understands the health of the person in their care. Care managers can also provide a placement assessment to an adult family home.
